General Information

Appointments - Our office is open from 8:00 a.m. until 5:00 p.m. Monday through Friday.  Our phone hours are from 8:30 a.m. to 12:00 p.m. and from 1:00 p.m. to 4:30 p.m. Monday through Friday.  Please call our office to schedule an appointment.  Our appointments are scheduled according to the type of problem you have.  When making your appointment, our staff may ask you about the reason for your visit.  Sharing the nature of your visit with us will help us schedule enough time for your needs.  We always keep time available in our appointment schedules in order to handle acute illnesses and accidents.  For urgent matters, we are usually able to offer an appointment either the same day or within 24 hours except on weekends and holidays. Because of emergencies or unexpected complications, our doctors occasionally run behind.  We are aware that your time is valuable and we will do everything possible to honor your appointment time.  We believe in giving time and attention to every patient.   Established patients are asked to arrive at least 15 minutes prior to their scheduled appointment time to complete the registration process.  NEW patients are asked to arrive at least 30 minutes prior to their scheduled appointment time with a picture ID.

Cancellations - If you are unable to keep your appointment, please call and cancel no later than 24 hours in advance.  While we realize that unexpected changes may occasionally cause you to miss an appointment, we reserve the right to charge you the full value of your scheduled visit if you do not keep your appointment or do not cancel in a timely manner.

Phone Calls - Our triage nurses are trained to answer any medical related calls for our established patients.  Please provide a daytime telephone number at which you can be reached and your call will be returned as soon as possible.  It is important to all of us that we give you our full attention when you are in our office.  Therefore, our telephone nurse will return routine calls after the morning or afternoon office patients are seen.

Referrals - Please allow 72 hours for non-urgent HMO referrals.  Insurance companies will not process same day referrals that are not emergent.  Please provide the name of the doctor or facility, the reason for the referral, and a daytime telephone number where you can be reached.  Urgent or emergent referrals are usually completed within 24 hours of the request.  

Prescriptions - When your medicine refills have been used, it is time to make an appointment so that we can determine how your medication is working and how you are doing.   Please call several days early to schedule the appointment.  Pharmacy Prescription refills are best handled by having your pharmacy call our special line.   These Prescription requests will be handled within 24 hours of receipt.  Your medical record must be reviewed before refills can be authorized.  We will not refill prescriptions after hours or on weekends..  We are unable to mail or fax prescriptions.

Medical Records - Our office maintains a chart of all your medical information.  This record is confidential.  No information in that record will be released to anyone without your written permission.  If you need reports sent to someone, you must let us know in writing.  If you move and want to transfer you records, you may receive a copy from us.  However, it is easy to lose records in the confusion of a move.   Usually, people find a new physician in their new home and that physician requests copies of your medical record.  We will be happy to send a copy of your medical record to your new physician after you sign a release. 

Payment Policy - Payment is required at the time of service.  This includes any copayment, coinsurance, or deductible required by the insurance plans with which we participate.  Our office accepts Visa, Mastercard, American Express, Discover, cash or checks for your convenience.  Our office files insurance on health plans with which we are contracted.  We are unable to file secondary insurance, however we will provide paperwork needed to file the claim upon request at the time of service.
